A KEY QUESTION
Ask what preservation aims to achieve at this site, what material would be used and what it cannot guarantee about later treatment.
What changes after extraction
The socket is the space previously occupied by the root. Its surrounding bone remodels after extraction, changing the ridge’s height and width.
Explain whether a replacement tooth is already planned or still undecided. Extraction and replacement are linked decisions, but one does not automatically settle the other.
The purpose of preservation
Also called alveolar ridge preservation, the procedure may involve graft material and a covering membrane. It aims to limit ridge changes, not freeze the original shape or guarantee an implant-ready site.
The clinician should explain the intended benefit for this socket and how it relates to your future replacement options.

Graft and membrane origins
Bone-grafting materials can come from the patient, another human source, an animal source or a synthetic source. Membranes also have different origins.
Raise personal, religious or ethical concerns before consent. Request separate names and explanations for the graft and membrane, plus any reasonable alternatives. “Bone substitute” does not necessarily mean synthetic.
Limits of the evidence
The Cochrane review, which searched evidence to March 2021, found substantial uncertainty. It did not establish a dependable reduction in later augmentation needs or a clear clinically important advantage for one material.
Ask whether newer evidence affects the recommendation. If avoiding another operation is your priority, discuss the remaining uncertainty explicitly. Further grafting can still be proposed.
Reassessment and alternatives
The site needs reassessment before later treatment. A bridge, denture or accepting the gap may be alternatives, depending on the clinical situation and your preferences.
If you later decide against an implant, what purpose would preservation still serve? Clarify that before extraction, along with any temporary tooth arrangement.
Records and review
Keep the tooth position, extraction date, graft and membrane details and planned review. Take home procedure-specific wound-care instructions and a clinical contact. Appearance alone cannot establish whether preservation achieved its intended aim.
Ask whether the treating provider offers socket preservation. Your dentist must assess suitability and reassess the site before agreeing a final implant schedule.
