A KEY QUESTION
Ask the clinician to compare the whole reconstruction and its longer-term uncertainties, rather than comparing fixture length alone.
What “short” describes
Length refers to the implant body within bone, not the visible replacement tooth. Research uses particular definitions of “short”; these are study categories, not dimensions for patients to prescribe.
Have the dentist show the site and explain what limits the available height.

More than bone height
ITI identifies local anatomy and individual clinical circumstances as relevant to implant length. Nearby nerves, sinuses and other structures matter, alongside the planned replacement teeth.
A scan’s height measurement cannot settle the whole decision. Discuss the space for the restoration, the proposed support and the bite.
Compare complete treatment routes
A grafting route may add procedures and recovery requirements. A shorter-implant route can have different restorative and longer-term considerations. Neither is automatically preferable for every site.
If both are reasonable, request outlines covering procedures, temporary teeth, review stages and what could change. Include any suitable non-implant alternative in the discussion.
Short-term and ten-year findings
A BMC Oral Health systematic review published on 1 June 2026 found lower ten-year survival for short implants than longer implants in the included studies. Limitations included small samples, differing study designs and people lost to follow-up.
The older ITI consensus concerned shorter follow-up and already raised concern about outcomes changing with time. Neither source supports equal lifetime performance or predicts your individual outcome.
Loading and the replacement tooth
Choosing a shorter fixture does not establish when it can support a working tooth. Placement, stability and loading need separate clinical decisions.
Find out how any temporary tooth is supported, who approves the next stage and how the finished restoration will be cleaned. Discuss what further care might involve if problems develop later.
Record the reason for the choice
Keep the recommendation and its reasons with the proposed restoration, appointment stages and review arrangements. If an option is ruled out, request the clinical explanation.
Ask the treating provider which short-implant systems and grafting procedures are available. Confirm the dentist’s recommendation and written plan before committing to travel.
Read the related guidance
Keep the wider treatment plan in view while comparing these options.
