A KEY QUESTION
Understand the outlook and alternatives for each tooth before agreeing to extractions.
Separate removal from replacement
Check whether the plan replaces an already toothless arch or removes teeth you still have. Request a tooth chart. Explain priorities such as preserving teeth or reducing surgery, and ask whether retention has been considered.
The outlook for each tooth
The dentist should assess damage, gum and bone support, restorations and maintenance needs. A photograph is insufficient. Ask which teeth are maintainable, doubtful or unlikely to be restorable, and what could clarify uncertainty.
- What is the diagnosis for each proposed extraction?
- Is the problem tooth damage, lost support or both?
- Could treatment or a specialist opinion change the outlook?
- What would keeping the tooth involve?

Ways to retain teeth
Depending on the findings, options may include gum treatment, repairs, restorations or root canal treatment. They cannot save every damaged tooth. Ask whether disease treatment and reassessment should come before a final replacement plan, and discuss the burden and uncertainty of preservation.
Compare the full treatment paths
There may be options between keeping every tooth and removing them all: selective gap replacement, tooth-supported work, dentures or implants. Suitability depends on the number, position and condition of remaining teeth. Compare temporary teeth, preparatory treatment, cleaning, reviews and what happens if a tooth or implant later develops problems.
When another opinion helps
If the reasons for removing all teeth remain unclear, request findings and imaging for an independent opinion. A dentist with suitable restorative or gum expertise may help clarify a disputed outlook. Share the full proposal and your goals, and allow time to review the answer.
Decide before travel adds pressure
Ask what happens if the examination changes the outline plan: can you defer extraction and consider alternatives? Check payment and travel consequences before booking. The treating clinician must explain each irreversible step. Keep the agreed tooth chart, alternatives and temporary replacement plan with your records.
