A KEY QUESTION
Ask the organisation holding the record for access or a clearly described correction, and keep its response.
Identify the record holder.
Start with the practice or hospital that provided the care. A scan, specialist opinion and general dental notes may be held by different organisations. Find out who handles information requests.
An intermediary such as Best Smile Turkey may hold booking correspondence without the full clinical record. The treating provider is the starting point for treatment notes.
Specify copies and verify identity safely.
Give treatment dates and the items required, such as notes, images and reports, consent records or a referral. If another clinician needs the files, establish a usable format.
Use the organisation’s secure identity-checking process before sending identity documents. Keep your request and its acknowledgement. Avoid posting health information publicly to attract attention to a missing response.
Explain the disputed entry.
Identify the dated entry, what appears wrong and the correction you want. Include relevant evidence privately. A contact-detail error differs from a disputed clinical description; a tooth number may need checking against the original chart and image.
Do not alter your copy and present it elsewhere as an amended clinical original. Request a response explaining what was accepted or why it was declined.
Keep later findings distinct from earlier opinions.
An earlier opinion can be accurately recorded even when a later examination reaches another conclusion. The later finding may need adding with its date and source rather than erasing the history.
UK GDC standards require identifiable, dated amendments. Establish how an accepted correction will appear and whether your concern can be recorded if disagreement remains. A request does not guarantee deletion.

Use the relevant UK or Turkish route.
The UK Information Commissioner’s Office explains challenges to inaccurate or incomplete information. Its guidance is under review following legislative changes, so check current requirements rather than relying on an assumed deadline.
Turkey’s data authority, KVKK, describes requests to the data controller, including rectification. Obtain the Turkish provider’s current route; UK guidance does not replace it. Keep the request, evidence and response for any further complaint. Do not delay urgent clinical care while a records dispute is unresolved.
