A KEY QUESTION
Make temporary work, outstanding treatment and the next responsible clinician clear.
Arrange the handover early
Ask who prepares the summary, which records are available and when you will receive them. Find out what the receiving dentist needs. GDC record standards apply to UK professionals; ask an overseas provider about its own process rather than assuming the legal duties are identical.
Match treatment to teeth
Request the clinician’s tooth notation with a plain-language explanation where needed. Have unavailable or irrelevant details marked clearly. This checklist is a practical request, not a list of legal requirements.
- Patient identifiers, treating dentist and clinical contact
- Assessment date, findings and diagnosis
- Procedure, tooth, date and relevant materials
- Completed, temporary, planned or declined treatment

Keep the supporting files
Ask for relevant dated images and reports, with photographs or laboratory documents where available. Preserve original files with clear names. A cropped screen photo may lose useful information. Implant details do not replace records for other dental work.
- Image date, area and report
- Procedure notes and relevant material details
- Consent discussions and changes to the plan
Identify unfinished care
Record temporary restorations, pending work and checks needed before the next stage. Include complications and advice given. Keep aftercare and medicine instructions in their original form; ask about unclear wording rather than paraphrasing doses.
- Outstanding procedure, tooth and proposed timing
- Review task and expected clinician
- Clinical contact and urgent local-care instructions
Check privacy and access
Verify the receiving practice’s address and secure transfer method before sharing health records. Keep a private copy. Open files and check names, dates and attachments before leaving. Request a usable language or agreed translation. Ask the originating practice to correct suspected errors rather than editing the record yourself.
Confirm the next appointment
Receiving records does not mean a dentist has accepted unfinished treatment. Confirm the purpose, any assessment and fees. Keep the treating team’s contact details available for questions. Clinical instructions, financial terms and travel documents may all be useful, but each serves a different purpose.
