A KEY QUESTION
Record what happens next, who arranges it and where the original documents are kept.
Review and records planner
Record the dates your dental team supplies. This does not set recall intervals, make appointments or send reminders.
Your entries stay in this page’s memory. They are not saved on this website or sent to Best Smile Turkey, a dentist or analytics. Leaving or reloading can clear them. Download or print only if you want a copy, and protect it on shared devices.

Give appointments a clear purpose
Use separate rows for routine reviews, postoperative checks and cleaning visits. NICE advises individual routine review intervals that are reconsidered over time. Copy dates supplied by your team rather than assuming a six-month or annual schedule.

Assign the next action
Name the responsible person or practice and choose a status. Use notes to explain whether something needs arranging or is already agreed. Mark only that row’s action complete: receiving records, booking a visit and receiving care are different steps.
Make documents easy to find
Record the location of treatment summaries, aftercare instructions and relevant implant details, with originals and missing items identified. Ask the provider how to obtain records. This tool takes notes, not uploads; a private-folder description is enough. Avoid credentials or unnecessary medical history.
Update after changes
Use notes for access difficulties, cleaning questions or changes to discuss with the team. Do not alter prescribed care yourself. New symptoms need advice rather than waiting for the listed review. After a visit, update the next step and date instead of carrying an old interval forward.
Save a private summary
Dated items appear first, with undated tasks last. The open count reflects your selected statuses, not oral health. Add reminders separately in a system you use. Entries remain in page memory; download or print a copy if useful and keep it private. Retain the provider’s original records.
- Identify each appointment’s purpose
- Name who arranges and provides care
