A KEY QUESTION
Know what will be used, who will see it and how to change your permission.
Care, teaching or publicity?
Images may document findings, help plan treatment or monitor change. Teaching, research and promotion involve other purposes and audiences. Establish whether each image stays in the clinical record.
You might accept record photographs while declining social media use. NHS photography guidance illustrates these distinctions; another provider’s forms and obligations need their own explanation.
What the form covers
Read the purposes, images and organisations named. Does it include video? Does permission extend to an intermediary or marketing agency? Broad wording such as promotional purposes needs explanation.
Request separate choices for different uses and keep your completed form. UK ICO guidance emphasises specific choices; do not assume every form or country uses the same rules.
Who could recognise you?
Your name, voice, treatment description or social profile may accompany an image. Cropping a photograph to the mouth does not necessarily make it anonymous.
Find out whether the audience is a private teaching group, a public website, paid advertising or social media. Permission for one audience should not be assumed to cover another.

Who holds the images?
Identify who stores the clinical originals and any marketing copies. Obtain the privacy information, retention explanation and contact for questions. Send sensitive material only through verified channels.
If you send planning photos through Best Smile Turkey, ask who receives them, how they are stored and which privacy policy applies. Check the coordination team’s role separately from the treating provider’s.
Changing your mind
Get the withdrawal contact before publication. A request should identify the images, uses and channels concerned. Keep the response and clarify what can happen to material already published or reposted.
Withdrawing publicity permission and retaining necessary healthcare records are separate issues. GDC and ICO sources explain UK expectations; an overseas provider’s legal position needs jurisdiction-specific advice.
Make the choice explicit
You can say: I am willing to discuss photos needed for my care, but I do not agree to promotional use. Have that preference recorded. If you also decline clinical images, discuss how that affects assessment.
For a discount linked to publicity, read the terms and consider the privacy trade-off. Do not rely on a verbal reassurance that conflicts with the form.
