A KEY QUESTION
Match the teeth and goals, then ask about the findings behind each recommendation.
Treatment-plan comparison
Put each dentist’s explanation alongside the other. Mark the questions you still need answered; this tool does not choose a treatment.

Compare the same decision
Name and date both plans. Add one row per tooth, area or decision. Note whether each proposal addresses pain, appearance or another goal. Copy clinical wording where possible and flag unfamiliar terms for explanation.

Record the reason
Put the proposed treatment beside the finding used to justify it. Note what could explain a difference, such as examination findings, images or your priorities. The sheet cannot interpret records. A useful question might be: “Why a crown rather than a smaller restoration here?”
Ask about alternatives
An option missing from a quote may not have been assessed. Ask about benefits, risks and the effects of monitoring or declining treatment. GDC consent standards support these discussions for UK care; overseas regulation differs.
Use the unfinished-question list
The tool gathers unfinished rows. Mark one discussed after recording the explanation; this does not record consent. If advice still conflicts, ask whether an independent clinical opinion would help. Keep the original plans alongside your summary.
Save and share carefully
The treating dentist must explain clinical recommendations. This tool cannot establish suitability or service availability. Entries remain in page memory and nothing is sent automatically. If you download a copy, keep it private and use a clinician-agreed secure sharing route.
